How do you get Hives and how do you get rid of them.?

I think i have hives but im not sure how i obtained them, im also not sure how to get rid of them. They've been disappearing and reappearing for the past 2 months, And they itch like crazy. Please give me a solution to get rid of them and also a reason to how i got them.
